ABSTRACT

A 65-year-old male patient, who had a history of chronic lymphocytic leukemia for 3 years, presented with erythematous swelling of the right cheek for 20 days and scattered papules on the back and upper extremities for 10 days. Twenty days prior to the presentation, the patient was hospitalized for disseminated herpes zoster. Skin examination showed diffuse dark red swollen plaques in the facial area under the right eyelid as well as on the right auricle and external acoustic meatus, with a sense of infiltration on palpation; scattered brown crusts were left behind at the sites of healed herpes zoster lesions, and scattered depressed scars were observed among these crusts; scattered infiltrative, mung bean- to soybean-sized, light red papules with a smooth surface were seen on the back of the neck, back and upper limbs. Histopathological examination of the facial skin lesions revealed nodular infiltration of epithelioid cells, lymphocytes and many multinucleated giant cells in the dermis and subcutaneous adipose tissue; immunohistochemical staining showed positive staining for CD68, CD20, CD79a, CD3, CD2, CD10, CD5 and Bcl-2, scattered positive staining for Ki-67, and negative staining for CD23, cyclin D1, Bcl-6, multiple myeloma oncogene 1, CD21, CD35 and myeloperoxidase. The patient was diagnosed with Wolf′s isotopic response manifesting as granulomatous inflammation after disseminated herpes zoster. The patient was treated with intravenous drips of methylprednisolone at a dose of 40 mg/d, and the skin lesions were gradually improved and subsided. No recurrence was observed during 4 years of follow-up.

ABSTRACT

Integrated experiment course is beneficial to optimizing the whole structure of medical courses and plays an important role in improving medical students' comprehensive quality and innovation capability. After several years of reform and exploration, Shanghai Jiao Tong University School of Medicine has developed the medical functional integrated experiment course with concept of "strengthen quality, reinforce foundation, encourage innovation and provide professional service", established a diversified evaluation system, and developed a corresponding assessment scale for reviewing students' comprehensive ability and the quality of experiment teaching. This paper investigated and prospected how to construct and optimize this evaluation system from the aspects of the relationship between evaluation method and course structure, and their effects, so as to provide strategic guidance for the further development of integrated experiment course.

ABSTRACT

To cultivate high-quality medical talents,Shanghai Jiao Tong University School of Medicine established a multidisciplinary and comprehensive basic medical practical integration curriculum Medical Functional Experiment.Over several years' exploration and renovation, insisting on student-centered,this course has established a modular experiment teaching system with emphasis on foundation,combina-tion with clinical and focusing on innovation.Integration curriculum adopts four-stage teaching pattern which is suitable for medical students, and implements diversified teaching method and evaluation system. And meanwhile, the paper explores the ideas and approaches to further deepen the construction of teaching staff, expand experimental teaching techniques, strengthen the construction of curriculum materials, and strengthen the cultivation of teachers' innovative ability.

ABSTRACT

The transcranial doppler ultrasonography (TCD) is a non-invasive detection methods of evaluating intracranial artery.Since the 1980s,TCD has been extensively used in various fields of clinical work.Because of its simple operation, good repeatability, and the continuous bedside observation of patients,TCD is especially suitable for severe patients.Increased intracranial pressure is one of the important reasons for the deadly disease in children,it can make the cerebral blood flow perfusion decreased, causing serious consequences, such as brain dysfunction,so intracranial pressure monitoring has important clinical significance.TCD as a noninvasive monitoring tool, can monitor the patients with increased intracranial pressure dynamically, according to the blood flow velocity, the related parameters and the wave of cerebral hemodynamics,so as to achieve the purpose of monitoring intracranial pressure change. This article focused on the TCD application progress in several common children's diseases of increased intracranial pressure.

ABSTRACT

Objective:To explore the roles of insulin-like growth factor 1(IGF-1) and tumor necrosis factor-?(TNF-?) in diabetic reti nopathy. Methods:The serum levels of IGF-1 and TNF-? in 127 cases of type 2 diabetic patients (DM group) and 36 cases of health control (control group) were detected by ELISA method. Results: (1)The serum level of IGF-1 in DM group were lower than that in control group(P
